They assess the individual's needs, develop a personalized care plan, and coordinate and monitor the implementation of the plan.

 

Senior care managers communicate with healthcare providers and other relevant parties, provide guidance and support to families in making informed decisions, and identify and access appropriate resources. They act as advocates for their clients, ensuring their needs are met and wishes are respected, and regularly evaluate the effectiveness of the care plan, making adjustments as needed.

If your loved one is struggling with daily tasks, has health issues that need regular monitoring, or you’re finding it difficult to manage their care on your own, Part of the Family can help.
